Wir beleuchten in diesem Beitrag die demographische Sonderstellung Sachsens und wie sich diese auf den Pflegesektor und durch den Mangel an Ärzten auch auf die Gesundheitsversorgung auswirken wird.
Das deutsche Gesundheitssystem und die Sicherstellung der regionalen Versorgung stehen vor dem Hintergrund der demographischen Entwicklung vor besonderen Herausforderungen. Besonders die ländliche Bevölkerung Sachsens weicht im Altersdurchschnitt vom bundesweiten Wert deutlich nach oben ab. Diese Entwicklung wird sich gemäß der Altersfortschreibung dahingehend fortsetzen, dass im Jahr 2030 in einzelnen Regionen die nicht mehr erwerbstätigen Alterssegmente (ab 65 Jahre) einen Anteil von über 30 % der Gesamtbevölkerung ausmachen werden .
(Die angegebenen Werte je Jahr wurden als Durchschnitt aus zwei Prognosemodellen für einen starken bzw. schwachen Zuzug ermittelt.)
Mit steigendem Alter erhöht sich zudem das Risiko, chronisch zu erkranken. Das zeigen auch die zehn häufigsten Krankheitsdiagnosen bei Patienten, die älter sind als 65 Jahre und 2014 von deutschen Hausärzten behandelt wurden . Neben einer steigenden Prävalenz chronischer Erkrankungen steigt auch die Zahl derer, die dauerhaft auf Leistungen der Pflegekassen angewiesenen sind .
Diese dauerhafte Versorgung ist in Teilen Sachsens jedoch zumindest gefährdet. Bereits jetzt kommen im Landkreis Meißen auf einen Hausarzt knapp 2.300 potentielle Patienten, im Erzgebirgskreis sind es knapp 4.600 potentielle Patienten je Internist , also einem Spezialisten für chronische Erkrankungen etwa des Herzens oder der Lunge. Zudem sind mehr als die Hälfte der Hausärzte in Sachsen älter als 50 Jahre; unter 40 Jahre alt sind weniger als ein Zehntel .
Auch eine dauerhafte informelle Pflege etwa durch jüngere Angehörige ist nicht gesichert, da sachsenweit deutlich weniger Menschen leben, die dem Alterssegment der 18-30-Jährigen zuzuordnen sind als solche, die älter als 65 Jahre sind. Erstere werden jedoch die pflegenden Angehörigen der Zukunft sein. Zudem ist auch in Sachsen ein Trend zur multilokalen Familie zu beobachten, ausgelöst durch den Bedarf an höherer beruflicher Mobilität . Die Verfügbarkeit des informellen Hilfepotentials wird dadurch geschmälert .
Im Lichte dieser demographischen Entwicklungen sind somit innovative Versorgungsformen vonnöten, die einerseits den Zugang zu ärztlicher Versorgung unterstützen, andererseits den Patienten einen selbständigeren Umgang mit ihrer Krankheit ermöglichen. Telemedizin-Lösungen erfüllen zumindest auf dem Papier diese Erwartungen . Besonders das Selbst-Management chronischer Krankheiten wie Diabetes Typ II wird durch innovative Lösungen wie etwa Gesundheits-Apps erwiesenermaßen erleichtert , da diese auf einen aktiven Umgang mit Krankheit und deren Therapie beruhen .
Allerdings sind technische Lösungen nicht uneingeschränkt und ohne Vorwissen anwendbar. Ihre regionale Passfähigkeit muss ebenso geprüft werden wie ihre Akzeptanz durch das Individuum , sei es der Patient oder ein Gesundheitsdienstleister. Beides ist nicht als statisch, sondern als veränderlich zu betrachten. Zum einen ist anzunehmen, dass technische Kenntnisse der Endnutzer durch die steigende Nutzung des mobilen wie stationären Internets ansteigen werden. Zum anderen sind infrastrukturelle Reformen ein Kerngebiet des ausgehandelten Koalitionsvertrags zwischen CDU und SPD .
Damit die Verbreitung sinnvoller technischer Lösungen auch im Gesundheitssektor voranschreiten kann, wird unser Forschungsprojekt Care4 Saxony Hemmnisse untersuchen und Lösungsstrategien entwickeln, um den Übergang innovativer Telemedizin-Ansätze aus der Pilotphase in die Regelversorgung zu erleichtern.
